Stock refreshers are formulaic based on target compensation for level and performance. So if you get meets all expectation you refreshers are = (1/4 target equity comp) * perf. Target equity comp is what new hires at your level get for equity over 4 years. Perf =1.0 for meets expectation, .75 for meets most and 1.25 for exceeds expectation. This is designed so that your stock compensation stays on target as years go on

Google removed first year refreshers as of 2018 new hires. Still get refreshers for later years though Facebook has refreshers from first year on, formulaic and scales by performance
Yes, unless you receive a Needs Improvement rating. I received $150k on my last refresh as an L4 with top ratings.

can't say for US, but for IDC the leadership has supposedly done the math and figured out that they can go for lateral hire with a lower comp rather than give meaningful annual refreshers. They do try to provide SSA to folks who'll be seeing a cliff - but it's just not good enough to match Google, Facebook, Uber etc. numbers.
