I have a hard time asking hard questions over the phone since (1) it's harder for me to assess the candidate when they're only answering verbally (without whiteboard) and (2) they can always cheat.

It's not easy to tell when they're legit writing vs cheating.
I got a leetcode hard for the phone screen (checked after the call), in the on-site some were easier, some were harder.
I ask the same question in both, but my bar is lower for phone.
