Do recruiters have to ask about the visa sponsorship to every candidate?

A recruiter asked me today and I think it’s somewhat funny because I’m a native Californian with no accent so I wonder if that’s normal or not.

I don't see problem asking. It's a waste of time and money of the company if the company cannot sponsor visa but the candidate needs it. So it's better to ask a possibly redundant question than not.

It could be for a Perm position, where they have to filter only citizens or gc holders.

Yes. A lot of immigrants have lived in US for a long time an have better English and accent than many natives. Are you making the assumption that immigrants can't speak like natives?

Almost all of immigrants who migrated after high school years have accent. That includes British and Aussie :)
