Points/Miles :- I do have credit cards that have points/miles/rewards but whenever i need to fly for family or personal reasons or can fly for vacation tickets are way higher than the points/miles I have 🙄 never been able to book an international economy let alone business thru my points / miles . Yes. I pay out of pocket when flying with family . Spend about 1.5--2 % of annual AGI for round trip tickets for the entire family for each trip. Maybe 2 trips a year. I never have had to travel for work. Used BA, United,KLM, Emirates,Air France, Singapore, Qatar. I avoid air India, air China, and Turkish. Poor reviews. Google for reviews. There are tons of detailed review websites for how to score deals and for cabins reviews. Don't buy without some research. Prices vary widely and can swing wildly. Unless the money is chump change to you.

No but I generally get eco+ and buy upgrades when possible and it's a 9 hours+ night flight. The price difference is too hard to justify otherwise, you can just get such a nicer hotel and or restaurants. TC : 250K, would probably start buying straight business at 500k
Sometimes, other times we buy first class tickets. It depends on price and distance.

Never business. The price simply don't make sense until you make 1M+ a year. I'm fine having a bit of a worse travel for 10 hours and take a quick nap on arrival in exchange of saving 10k$. That being said, I upgrade my personal bookings once in a while with the miles I save from traveling for work.
Maybe we think differently... most of my family thinks like you too ... but I feel spending <=5% on comfort + travel is okay ....
The amount in actual money is so huge that it really doesn't make any sense. Do you really value sitting in a slightly better seat for 15hours worth 10k$? I guess if you value it then fine. I mean I wouldn't even pay 1000$ for it and I make 400k+ TC.
You never pay for travel out of pocket. That's what miles, points, and upgrades are for. I only fly business.
