Accidentally, intentionally? What happened? Is getting “blackballed” in the industry a real thing?
I gave an honest review about a team member who wasn't pulling his weight in projects, and just had the weirdest careless attitude about everything we do, he was gone in few weeks after, my guess his reviews weren't positive anyway, but I'm glad I did my part.
I got somebody fired by refusing to do their job.

Yeah. I would work hard to get ahead as a sales analyst and this other guy kept lagging behind and not being there during quarter end. I picked up his slack for a few months but got sick of it. I talked to my manager and they had given him some warnings but he didn't improve. I got tired of his excuses so I documented his mistakes and brought them to our boss. They let him go.

Management was looking for an excuse to let him go. And his mistakes cost them a couple of grand and were things that showed he wasn't following through on his day to day work. They didn't seem to mind. My coworkers were also picking up his slack. They weren't initially happy about it but forgot about in a few weeks.
I made a bug. QA guy signed off without even trying it. Released, shit hits fan, QA guy got removed.
That is diabolical
It wasn't deliberate on my part. I also did not cry when I found out though.
