Also, with regards to the annual cash bonus, how challenging is it to hit the 200% (exceeds expectations) threshold? Weighing a FB offer and clarity on these questions will help me make my decision. No. It would double the target cash component, which is 15% of base.
Honestly, you should not expect GE (greatly exceeds expectations, x2 multiplier) or RE (redefines expectations, x3). This is usually not a sustainable state, especially for higher levels. If you receive these consistently it means you are underleveled to begin with. Its still great to have the ability to hit GE or RE in a one specific half, since you will be rewarded for your effort if you were killing it. Also, the ratings influences your bonus and your refresher. So if you get GE for two halves at E5, you would get: annual salary*0.15 (target multiplier) *2 (performance multipler). But you would also get x2 the refresher!

No. Here is an example with real numbers. Assuming you are E5 level, and you had GE two halves. For example, 100k salary. Target bonus for E5 is 15%, so you would get 15k for a meets all rating. With GE you would get 30k. The actual formula is a bit more complicated (it includes company multiplier), we do perf review twice a year, and the multiplier only applies for the amount of money you received thst half.
