I did both. I got letters from my two previous supervisors personally, one of whom didn't even work for the company anymore (so that letter wasn't on company letterhead) and from one that did (so that letter was on company letterhead). I also got an official letter from HR confirming my title and total tenure, since I only worked for those two supervisors for the last part of my employment. I included all the letters in my filing, and never got any question about any that during the interview.
HR won’t give your roles and responsibilities but they give company letter head with minimal details. Get that and also get a detailed letter with roles and resp from any of your previous team mate or previous supervisor. Submit both and that should be sufficient!
You may be able to coax HR into including the official job description for the role, such as confirming the public details in the ad for the job you accepted. Maybe.
You need to get skills verified 'verbatim' on your EVLs. HR usually only signs off on the official letter(with the required company letter heads etc.) after a manager who has supervised you in the past signs off on your skill(s). This is the part that took around 9 months for me because one of my old managers gave me a lot of trouble signing off on it. BTW, you can also get your skills verified from your grad school or higher professors if they have directly supervised you.

For employment verification letter, the HR is enough. However, to verify specific skills, you would need your old boss.
Not necessarily. Even the HR can give the specific skills verification letter.
Could. But usually won't. Most HR will only confirm title and job description. Job description MAY be enough for the skills but may not be.
