So I had an on-site interview last week that went great with a small start-up company. They told me they'd have a decision by this week and today they asked if I could have a phone interview in Wednesday with the CEO. Do start-ups normally do this? If they do they do it for more than one person or are they most likely going to offer me the job?

It's usually a good sign. Sometimes just part of the process. I think of it like this: the hiring manager or HR wouldn't want a complete idiot to meet with the CEO, makes them look like the idiots.

Startups don't "normally" do anything. They're all making it up as they go along, and there's no generalizing. It is a good sign they want you to talk to the CEO. If they didn't want to hire you, they'd tell you by email (if you're lucky-- sometimes it's just silence).
Small companies will have you meet with the CFO and/or CEO, because they still make the major hiring decisions. That's how I got my last job. It's actually awesome, you talk to the people running the business versus some clueless HR person.

I was interviewed by the CEO of a startup ages ago for an internship there. No biggie.
They are trying to sell you. This is good. Every pre-IPO startup I've worked at or applied for included a chat with the CEO. Regardless of my level. Any where from new college grad to senior director.
