I signed the page and sent it via UPS to lawyer, but UPS lost it and they are not able to find it. Anyone faced anything like this? Still waiting for reply from lawyer.

Yeah and it was guaranteed delivery. It really sucks. It include cashiers check also of 1410. Now I will have to deal with bank for lost check as well.
Your lawyer could ask DOL for duplicate, but this really sucks. UPS is a horrible service

Well, things like this happen sometimes. Don't panic. On Form I-140, there's a box you can check if you need USCIS to request a duplicate labor certification from DOL. It will delay the processing, but you should still receive the original priority date (the date when the PERM was filed).

I checked the form again and thanks for explaining that. Only thing, I am worried they should not reject it because of duplicate.
Thank you all for your comments. I talked to lawyer and they said, they can still file i140 with signed copy, but will not be able to do premium processing.
btw, when this happened to me in 2016, we filed a premium processing request and it was rejected initially (I guess because of the PERM issue). But then later they told us we could reapply for premium processing. I guess it was because the PERM was received.
Ohh thank you so much for your reply. So, its possible they can still approve i140. I was worried that they will reject it in normal processing also. This gives some relief.
UPS found my package and delivered today after 1 month. My lawyer was going to file regular processing this week. Glad they found it.
