$55/day? That's not much for major US metro areas. You'll be eating pizza and burgers at that per diem. Btw, AT&T international day pass is $15/day abroad which includes voice, text and data. Worked great in Mumbai this past April...
Food expenses, yes. There's a per country cap documented on an inside page. I believe the cap for USA is $55 a day. I have personally not requested reimbursement for a SIM card but have heard it is reimbursement. In any case, check on inside, not blind.
Is there a cap even if food bill is included in hotel bill?
Generally hotel will also have a limit and hotels generally classify good separately. So food part may go against food quota.
