You can travel to any island safely. Coming back is the problem. If your Visa stamp has expired, you would need a new stamp to get in. Approved extension allows you to continue working legally, whereas you need a valid Visa stamp for broder crossing privilege.
Just to be clear. With an expired H1B you can't enter back inside the US without valid paper. So just go travelling within the US or arrange for stamping.
Yes you can. I did. From Canada. Not sure about Caribbean. As long as you have valid status and donβt stay more than 30 or some days I think, you can enter with expired visa. There are specific provisions that allow this.

While coming back from Puerto Rico.. agents do check for visa.. citizenship papers.. before boarding. Puerto Rico is a special case.. and some how they check for these. They also checked similar paperwork's while boarding for Vieques ferry launch ride.

I came back last week from Puerto Rico, and nothing was checked (except DL). So traveling to and from PR is like traveling within the US.
With an indian passport and expired visa, you are pretty safe. You wont get mugged or something. Everyone knows your pain :P (Bad joke, but couldnt resist it)
When you just wanted to go to West Indies but get deported back to the real Indies instead...
