      There are thousands of schools. Really good ones are competitive to get in but not as much as getting in for their under grad degrees.
      GRE, college grades/GPA, SOP and LoRs matter. Nobody verifies if you're in the top 10% or so. Universities either ask you to get your international college grades evaluated by third-party org or have their own database to convert your college scores to understand your competence.

    It is what you make out of it.
    Most classes in the US are absolutely graded. That is everyone could fail or everyone could get an A. So, competition doesn't matter a lot.
    Not, really. Grades are generally a reflection of the effort you put in and you don't have to really put in too much effort. It depends on you, though.
