Is there this possibility? Or, do the on-site interviewers try to avoid repeating the same LP questions? For example, a question about the most innovative thing that you've done... if you already answered this in the phone screen, and you're asked again on the on-site, then technical the answer on the on-site cannot be the most innovative thing you've done

No duplicates. Each interviewer looks for different LPs and they also go over questions in the pre-brief
I actually would say it is a possibility. The only way we know exactly what was asked is the notes from the phone screen. If the interviewer left shit notes (which happens) I’d say it could happen
There can easily be duplicates between on site and phone screen, especially if the HM and/or interviewer didn’t bother reading the PS feedback. However describe the most innovative thing you’ve done isn’t an LP. They could have assessed many (all?) different LPs based on that question. It’s unlikely someone is going to ask you that same exact question and even if they did they they might be looking to steer the conversation towards another LP. Back when I was at amazon we didn’t even have specific LPs that were assigned during the PS. At best we would talk about something kinda random and if something popped out that was LP related we would include that in our notes and possibly have it impact our decision for being inclined on the PS (though it would usually have to be really good or bad to sway the decision). It sounds more like that’s what this question was about.

I wouldn't even worry about such technicalities. If we were at the point where that was what we talk about when hiring a candidate or not, then we wouldn't have a hiring problem
If the hiring manager and recruiter are organized, there shouldn't be. Ideally they sync up ahead of time and each person knows what LP is covered in which round so there aren't duplicates and they get breadth on your responses.
