Has anyone ever failed team matching and didn’t receive an offer? I had one team matching with Capital one but now is asked to do another one.

I literally just went through that cycle with Capital One. Their team matching is a little weird and it's on a "team priority" basis. I was contacted by two teams showing interest in a span of two weeks, and in both occasions the recruiter came back with "the position was filled" or "they went another direction". My third team matching went well and I will be starting next week.
I wouldn't worry if I was you. Team matching isn't really an interview, it's more of "getting to know you and seeing if you fit the team", so I wouldn't worry about not matching with a team as of yet.
Good luck!
