Currently, it doesn't matter how knowledgable you are or what you work experience is. In 45 minutes, if you don't show off your Leetcode cramming performance your career is doomed.
True absolutely but there are some some companies which have more realistic approach towards interviews. Once such company I interviewed was thoughtworks.
What do they ask instead?
Interview will span full day and you have to do pair programming with one developer there. They will be monitoring your coding style , design and how you communicate as a person. Everything is done in pair programming mode which I really liked compared to random leetcode questions. This interviewing style is more closer to the day to day work that developer will be doing

Bullshit, nobody asks any damn design question in 45 minutes phone interview. I don't know if you do it differently at Pinterest.
Not in phone interview. However, in onsite interview there is a dedicated 45 min interview slot for design. This is standard in pretty much every place I have interviewed.
