One big tell that an org has problems is an overall decrease in transparency. Can people talk about things and hash them out in the open? Or does every conversation need to be something had in an office or conference room? If every discussion or meeting happens behind closed doors that's generally not a good sign. You don't need privacy for good news.

the internal signs of a company failing? Long term revenue trajectory. Competitors taking market share. Lack of new ideas or resistance to change and take chances, just running the business as usual. Relying on price reduction to keep customers. This can be strategic, but look out if it is reactive. Borrowing should mainly be to invest and grow. If cash flow is drying up, that could be seen in increased borrowing. #1) When projects are not getting funded across the company it is time to check out.

Levels of BS raise
Hiring in bulk, lack of transparency, hubris, condescension on newer and younger employees, hierarchies, empires, favoritism, decisions that don't make sense to anyone except the decision makers... I could go on, but any of the above is a sign of impending doom.
