No lottery. The h1b is valid for the time you spent in the US. It doesn't even count the days you are on vacation outside US. If you move out of the country, your h1b freezes. You can return whenever you want as long as visa rules don't change. You don't have to go through the lottery again. I know people who have done this.

No, any company is ok. Although the people I know mostly took internal transfers so it's easier to find the opportunity. What were your reasons to move to India then and back now?

Looks like you don't have a limit. There's a doc from BAL called 'AC21 Regulation summary'. Just Google it. Quoting "To be eligible, the individual need not be in H-1B status or in the U.S. at the time the H-1B petition is filed on his or her behalf. The individual must have been previously granted H-1B status and be otherwise eligible for an H-1B approval."
