You also start with 2 'floating' days that do not carry over year to year, but do refresh yearly.

Manager's discretion I believe. But work at the country club is already vacation enough.

At Microsoft, it is standard practice for managers to grant extra days at their discretion. It is handled by not entering those days in the PTO tool.
3wks/yr. Accrues every pay check bi-monthly.
Bi-monthly? Not bi-weekly?
