The management who decided to outsource the engineering. I couldn’t believe they did that when there are only a handful of countries in the world that can build airplanes. It was a disaster waiting to happen. Same thing in pharmacy industry which caused lots of death in Africa due to incompetent management for the quality in outpost.
The pilot solution to the specific flight scenario caused by this new system is the same solution that pilots should have been trained for in a very similar flight scenario. If a well trained pilot had encountered this situation, he would have turned off automatic trim and the problem would have been over. These pilots were not well trained (relative to standards at major airlines) and were not subject to the same level of ongoing training. When they encountered this trim issue, they screwed up. It was probably reasonable to assume that pilots would know what to do in this situation, despite the fact that it turned out not to be true. In the end, Boeing assumed that airlines who were more reliant on less trained pilots would opt for the optional redundancy, when it turned out that airlines too cheap to train pilots well, are also too cheap to buy additional safety features.
Dang I didn't realize that we had this close of a relationship with Boeing. I knew about the "won't sell a 737 below what SWA buys it for" but not the rest! Regarding the disaster, Boeing Mgmt, imo, definitely has blame. But at the same time, the media definitely has a penchant for playing up airline disasters way more than say...GM's exploding ignition disaster or the fact that we have thousands of deaths on the road due to stupid drivers, terrible roads, and overall very lax regulation in the auto industries compared to airline.

I would argue that everyone is to blame. Even the people who don’t work at Boeing and have nothing to do with airplanes. No one prevented the crash so everyone is guilty of not preventing the crash
