Does the tution fees of every university gets waived off if one work as RA or TA? EDIT: Can anyone name a few?

yes if you have atleast 1/2 time appointment

Varies. For some programs they only do tuition waivers and stipends for PhD candidates (not masters)

Yeah...in most state schools it is waived or converted to in-state tuition ( which usually is about a third of out of state tuition). For private schools, mostly no.
