The doctors don't know what's wrong. I'm scheduled to get an mri but insurance is taking two weeks to approve. It's been six weeks of feeling tired and mildly sick. I've been taking a couple of sick days here and there.
This has affected my output and my manager is calling me out on it saying my next review could suffer. He knows I've been less than 100% health wise and wanted to talk to hr about adjusting my performance requirements but doesn't look like it's happening.
Should I take long term leave until I feel better? Or stick with it while letting the manager know I can only operate at a limited capacity?
If I take medical leave, what do I say the reason is? My condition is still unknown.
How will this affect my review?

Take leave and take good care of yourself if you are financially ok. Hope you get better soon.
