He also says that 25% of the oxygen mask systems are defective, and that the planes had a large number of parts that were known to be defective at the time that they were installed, but that the company installed them anyway so they could meet their production deadlines.

This is what happens when you let the governing agency (FAA) be too friendly with those they govern (airplane manufacturers).
No, that is the final result. The onset of the problem is not paying government employees. A current FAA position in my area requires 15 YOE and pays 5k less than my 2 YOE HW Engineering job. That’s why the FAA can’t hire numbers or quality and must force the manufacturer to run the analysis.
