How does your team plan work? If sprints, who decides what goes in the sprint?
EM or tech lead should decide what goes into the sprint. PM, TPM decide on priorities and what the features are. PM, TPMs dont actually have to go to all sprint plannings.
A sprint planning meeting with PM, Dev Lead, Dev Team and often Dev Manager, all PM requirements for the quarter are discussed and based on priority, resources, time ... etc items will be added to sprint.

So who creates the workitems? And how do you cost them out (story points, hours, days, ??)?

PM creates Requirement, the team breaks down the requirements into epics, stories, tasks, milestones ... etc Sure milestones and epics especially need to be created in agreement between the PM and the Team, both are committing to them, and every story, task, bug, test, ... etc is listed under an epic, and is committed to a milestone (Fix Version). Story points are decided by the team initially during planning meeting, and team members update their story points estimate daily duirng standup based on their progress, and they may create new tasks to the backlog as more work is discovered
