Unused, Unstamped, Unexpired H1b

Company A filed my H1b petition in 2016 which was selected in lottery and approved. This was a consular processing and my employer never sent me to USA for project due to which the visa was never stamped on my passport. Now, can we file H1B transfer petition for such Unused, unstamped and unexpired visa for company B? if yes, how does it work? does it happen very often?

There's nothing like a 'transfer' of H1b. Change of employer petition is always a new H1b . I don't think you will be cap exempt, and I think you will have to go through lottery again.

I was in a similar situation. I was cap exempt.

They were in India, never used H1B. This was in 2016.
