How does someone report first hand accounts of unethical practices in hiring? I was told to personally not hire someone because they posted on social media they were about to go through medical procedures. The company's CEO stated that would cost the company money in insurance claims and for us not to bring that person onboard.

I think it’s only unethical if the person is a current employee. FMLA protects employees under certain criteria.

Incorrect. If the company is hiring, EEO applies, meaning they cannot discriminate based on age, medical status, etc. If they had said the rejection was based on absence during onboarding, it may fall into a gray area; however, I call BS there.

Assuming it's a US company or company hiring in the US, this would fall under EEO. You can find info on reporting here: https://www.eeoc.gov/youth/how-file-complaint

Note that it can take several months because, well, government; however, if it gets worse (obvious discrimination that you have proof of), you can anonymously report it to your local police department.
