I was approached by a startup recruiter and was told they allow 100% remote work. Does anyone know if h1b visa holder can permanently work remotely from outside US and still get paid here in US bank account?

Nope. It’s against the law. Employer needs to specify work place location and your US address as well. If you do; both you and employer would be at fault.

You don't need a US visa to work remote. You're not in the US, USCIS has no say in the matter so I don't understand your question. You need work authorization in the country to reside in where you are doing the work, not the US. You would presumably come to the US from time to time for business meetings. You would use a B visa, not an H, for that purpose--assuming it's just meetings and such and to are not doing significant work here.
