NI rating with lots of factual errors in action plan doc

I found a lot of factual issues in my perf and got an NI rating. I think my manager is a bit disconnected from things. Previously my manager had a bad reaction when I pushed back on his notes.
This time my manager shared a live doc that's also shared with skip manager. I am thinking of writing comments on the doc. Is this worth it?
I am also in talks with other team that wants me so this rating is a bummer and I am afraid it will screw things up for me.
Any advice?

It’s unfortunate your manager didn’t bring these up in 1:1 for addressing, bad form on them
