What do you think is wrong with the gaming industry?

I think after reading this alongside all the other sexual assault and harassment cases and that article about riot games, it's undeniable that there's a real issue in the gaming industry related to sexual harassment, assault etc. The handcuff stuff was really graphic...lol

What are the best ways companies could combat these issues? I feel like we can and should do better, but how? What is the crux of the problem?
