My friend was casually reading Elements of Programming without having a laptop to go through the questions in.

Can you “write” and debug code in your head? Is this an effective form of study?

It depends on what skills you are trying to acquire. Recognition of solution patterns is different from translating your approach into code, compilable and bug-free. If you just want to scan through some problems and think about possible DS and common techniques that can solve them, I think it can be beneficial.

There is no standard answer for what is and isn't effective. Maybe a casual read on a specific topic is exactly what he needs. Everyone has their own set of strengths and weaknesses when it comes to interview prep.
