Need some guidance on how to request for Short-term disability on mental health grounds.
Have been experiencing toxic culture for last few quarters and essentially working 18-20 hrs everyday. Stress has resulted in significant decline in health, happiness and hope. Additionally, my partner's job got impacted last month so it is an added stress as our survival in the US depends on my job (working on h1b). My company requires the doctor's recommendation to approve short term disability leave.
- Who should I talk to consider my short term disability? (e.g. HR, lawyer, therapist, etc.)
- how should I convince therapist that I need a break for couple of months to get back on my feet?
- What paperwork/evidence should I submit for the same?
