I mean, there's a thing called cost of labor. CA and NY get the highest, and the rest is naturally lower.
Depends on the company. Some put Seattle on the same pay tier (or very close) as SF and NYC, some don't.

Yea. I worked the retail side before i worked corporate and woooo boy. Dealing with some of the corporate people who would come in was just awful. A friend of mine worked customer service on the online store side and she would tell me horror stories of corporate apple employees who would call or chat in about their orders. Just very condescending, rude, treated retail like it was lesser. Edit: there were some who were very nice and open to answering my questions about working at corporate. Telling me to keep in touch if i need help etc.
Any satellite office is second class citizen in terms of perks, insurance and access to execs.
This is not the case in all companies, last job whole org was based at a satellite office.
