$800/year for fitness expenses. Could be for a gym/trainer or a pair of running shoes, just as ling as its loosely fitness related.
Health or "Wellness" center on campus with no copayments which include an optometrist, onsite blood tests, X-rays, chiropractor, massages, physical therapy, etc all without copays or heavily subsidized. The results are usually given within 30-mins or less for nearly all blood tests. The facilities are state of the art with doctors typically recruited from Stanford Medical Center.
We also get free drink tickets at some company events.
We have a VIP pass to the top of the Empire State Building. Best place to bring a date ;)
I hear a lot about Google free food. That's like thousands of free dollars.
Theres a lot that I think Blizz does for non-monetary benefits, but the most tangible has been time off for all departments between xmas and new years. Departments that can't shut down for logistical reasons get "special" vacation in equivalent hours to be used at a later date.

Free soda/drinks in every building. Some buildings have espresso machines as well. Our building also has an Xbox lounge which is pretty awesome during breaks.
oh yes, I have one small lobby right behind my office which has a 60 inch curved screen TV, Xbox one, Xbox 360 and several games for both. nobody plays except for kids visiting with their parents. and also me some days before coming home at night :D
I try to make an effort and invite people from our space during lunch. People feel bad for playing, but I think it's a great way to bond with your coworkers and get a break from your work.
$400 year "wellness" $ (bought an Apple Watch), great cafeterias and fitness classes, 6 month maternity leave :) I'm happy
