Planning to take FMLA for mental health in May. My therapist said it would be a good idea, but I’m not sure if they can sign the forms. Does it have to be my PCP or a psychiatrist(not seeing one yet)?

Everything. He messed up calculating the dates so after telling me 3 weeks he sent in a form asking for effectively asking for 2. Work calls me and tells me I've been no-show for a week and I'll be fired if I don't show up on Monday... Completely misinterpreted what questions were asking too. Like know how some questions say things "was unable to/currently unable to/will be unable to". Clearly that means "mark those which apply". But in his dementia riddled mind that meant all 3! So instead of answering "What job function can't be done" for example, this idiot answered not with a job function, but with a note explaining in the future I'd have no issues (because in his head his answer would apply indefinitely, so best to dodge the question...) And when the inevitable corrections were needed he dragged his feet until the 7 day deadline for corrections had passed. I mean this was a terrible provider, the last straw was him making fun of my adult virginity in a group session, but it seriously profoundly changed my life having the wrong person fill out that form. - Side note: I never even meant to send in the form he filled, if anyone @Google is reading, your decision to bury attachments on mobile got me fired. I had attached in a draft, but decided to ask my psychiatrist to fill out the forms. When I went to explain I'd need some more time the draft attachment got sent (since it's hidden under the email thread in Gmail's iOS app) and they insisted on using it since it was already "complete" Had I used the one my psychiatrist filled out I would have kept my job. There's never been a doubt in my mind about that.

I can't speak to Amazon's process, but at Microsoft the process is that the forms go directly to the short-term disability insurance provider (Prudential) for approval, and the Prudential just notifies HR with a yes/no without any additional details. When I went on leave due to anxiety/depression/ptsd, I had my psychiatrist and not my therapist fill out the forms because I had heard horror stories about Prudential making people go through multiple rounds of denials before finally approving and I heard that they were slightly biased to take a psychiatrist's recommendation more seriously. I'm not sure if I just got lucky, but my request was approved very quickly without any push back. Something to consider is that establishing a historical baseline that your provider can speak to may lend additional credibility. So seeing a provider one time for the reason of filling out FMLA paperwork may not be the optimal route.
Even with psychiatrist approval they approved only 30 days, I don’t know if they will even entertain therapist! They need medical records and treatment plan, which I don’t think therapist can do. So I suggest look for a psychiatrist
That wasn’t my experience. My psych sent in form with diagnosis and 12 week leave period; which was approved. If there was any need for treatment records, that was behind the scene and I was not aware of it.
yeah the way they wreck our brains they should. Actually they do have a new mental health benefit that started this April
Any good psychiatrist in WA state who helps with recommending FMLA for anxiety/depression? If anyone has experience working with a psychiatrist and successful with filing FMLA, please comment. Thanks
I have, but I'm pretty sure the main reason it was very easy for me to get it approved for short-term disability insurance payments was not the specific provider herself, but rather because I had a long documented track record of working with one psychiatrist over several years.
Yes they can, and please for the love of god make sure the more technically competent of your two providers fills it out. Have them both fill it out if you have to and choose the one that's more thorough/correct. My claim ended up being denied due to an incompetent therapist and it ended with a lot of pain for me.
