It seems like our company has a hard time doing either of these things. When we want to hire people we can't find anyone qualified enough to do a specific job and so we have to settle for mediocre hires or struggle without a hiring at all. When we do hire someone mediocre and they turn out unable to fulfill their role we struggle to fire them because we're such a small team and need as much help as we can get... Any other companies struggle with this issue? How does your company handle things?

Hire and fire until you grow bigger to offset any negative reviews in the past. Also fire mediocre managers and trainers. They might be the problem
I think that to hire great tallent you have to hire great leaders that will help bring good tallent. I have hired hundreds of engineers and everyone that interviews with us are amazed at how smart they are. To do that you need good recruiters, good sr engineer that can gauge the skills and a great manager that can sell someone. Also after the person is hired invest time into those people.
