1:1 is for you career growth and it’s your responsibility. Take initiative and have weekly 1:1. It’s way to align on performance expectations and career growth opportunities.
Everyone is different. I do at least weekly 30 min with new people, sometimes full hour if needed. More experienced self managed people I meet biweekly. Your manager might be expecting you to reach out and schedule them, but they sound like a bad communicator.

Just put meetings onto your managers Outlook. I too was ghosted by my manager early on, I was PIPed before 1yr.
Could be 2x a week during the first month or two, decreasing to once and then fortnightly. At least once a fortnight anyway. If manager keeps cancelling or rescheduling 1:1 then it’s a bit of a red flag. It’s supposed to be YOUR time. You can reschedule, but they shouldn’t be, or at least not frequently anyway.
It is a red flag. If you mgr has less than 10 person team, its a redder flag. Set up a 1-1 and in the meeting decide on the cadence going forward. You own the agenda then on.
