Yes. Your rating matter and a high rating will push you above the bottom of a level.

No. Two things will happen at the same time - promo and annual merit. The promo will move you roughly to the same percentile in the new band. The annual merit will pull you a bit further towards the middle of the band. So you can expect to be closer to the middle of the new band. That’s base salary. Refreshers are fixed per level, no bands there.

If promoted, you (generally) move to the same part of the band in the next level effective immediately of promotion. I was top of my band, promoted, and new pay was top of next band. You also get merit increases per year that vary but generally are between 3-7%. This affects your total cash takehome. Finally there’s equity. When promoted the majority of your RSUs will still be from your original grant. However, each year you get a refresher based on performance that is generally baseline about 20-25% of the RSUs that a new hire at that level would get when signing. So if you signed for 400k RSUs at E4, each year if you Meets All you’ll get 80-100k new RSUs. If you are promoted to a level that would get 800k RSU, at the next refresher if Meets All you would expect 160-200k etc etc. RSUs like bonus and merit increase also vary by your performance rating. TLDR you do get paid more, but you get paid less than a new hire at that level.
A somewhat minor detail - merit increase (for a given level) depends on where in the band you are. RSU refreshers don’t, they are an absolute value per level, with fixed/known performance rating multiplier. Bonus is fixed as percentage but will depend on base salary for the last year (review period).
Are refreshers prorated? Like if I was 4 for a half and 5 for half what would my refresher be?
Unsure about your question. However, if you’re still in the negotiation phase, ask for the higher level now. Don’t believe the whole “we’ll promote you soon”, “we’re putting you at the top of the band”, etc. I’ve seen them tell several people this, but it can take ages to get promoted and it’s best to just come in at the higher level.
