Hey anyone at Microsoft. I'm thinking about making a change to go back there, after 5 years. I'm wondering about my rehire status. Is there anyone who can check this for me confidentially?

It's most likely not checked. Per my recent discussion with HR, they only check that if you're fired for gross misconduct (e.g. stealing etc), not performance based stuff

My boss tried to hire a vendor as fte and was told by hr that the vendor was ineligible. He asked the vendor's last manager about it and was told that the guy was vsa'ed for low performance. Nothing egregious.
You would have been notified about rehire ineligibility during your termination. Notification is part of the process.
Problem is that I was in such shock that I don't remember what was said. I know at Amazon rehire ineligible only lasts a year unless it's due to an illegal act (or at an FC). I wonder if Microsoft is similar

Apply for a job. You'll know right away. Rehire block is generally for conduct infractions not performance related stuff. There are limited other cases as well but it doesnt sound like you fit those.

Are you a member of the alumni club? There are dedicated recruiters attached to the club who would probably help you out.
You can easily check yourself as I have done. just call the 800 HR number and ask! You have every right to know and they will tell you! Sometimes after a certain amount of years you can request to have the status from not re-hirable removed.
Microsoft recruiters are idiots. They may take all rounds and later realise that you are in no hire list. I don't have proof of that, but the recruiter ghosted me once after all the good rounds.
