And approval depends on the group. Some groups are perceived money makers and approve, some are perceived cost centers are regularly denied.
As with pretty much every question of this nature: it depends and, contrary to popular belief, common sense typically prevails. Nobody will force you into a cargo hold for your daily, twelve-hour flight.
Makes sense. My old company had a simple policy of business above 8 hours that only required approval if booked less than 30 days in advance.
Get a medical exception... if you have back issues or some other health problems that prevent you from being confined for an extended period of time
Common knowledge that nobody from Europe flying to Seattle requests cvp approval as it's not in the budget. With most not direct it will be roughly 15 hours flying
