Not really. If you read the discussion on slack you would understand it was not just the guy’s fault. There were checks and balances that failed.
“Going down the EBF route meant fewer approvals and a shortened process that wasn't gradual.” Still some approvals required. Doesn’t sound like a single person taking the whole thing.
Yes there were approvals by higher ups in management chain but nobody is going to hold them accountable. Its always the engineers who get shafted.
