Well... I can only say, seeing my teammates writing code without really understanding what's happening... or perhaps understanding it only 50% but still go ahead and do work on it. Another case was the classical guy... saying "BOOM BAM! AND it is done", and listening heavy metal and bouncing on his chair all day. He would push some code he "thought" is correct to all production servers without testing, and break the whole website for an hour, and see who is the intellectual, kind person in the team to be the scapegoat and to backstab. Is that "slinging code"?

Post mortem should tell you who pushed that code that broke stuff ! If you don't have a way to back track to who pushed the code , then you are doing it wrong
yes, post mortem, etc, etc. The CTO played Call of Duty with some guys... his "pet" is the guy that let him win. He is not going to use "post mortem" to fry his pet. By the way, it is not Apple... most of us worked in more than one company.
