they do cover gender reassignment, not sure what other elective surgeries. i don’t think unrelated cosmetic surgery is ever covered.

$130 for a family of 4. No deductible.
Intel has a high deductible plan where nothing gets deducted from ur paycheck... if u goto a doctor first 3500 you pay (you can use HSA) and then rest is covered...
