I have a iPhone 7 plus. Bought insurance through T-Mobile for $12/month with $175 deductible. Covers theft and accidental damage....good or bad deal??
3 years back I bought a $500 phone with $10/month insurance. forgot to cancel insurance after 1 year as I had planned. phone fell and broke after 1.5 years. best buy did not have a new phone to replace my phone. they refunded me $500 in best buy credit. so insurance paid off. with my current phone I don't have insurance.
Bad idea. For people with good salaries like you and me, you need to self-insure for anything that wouldn't put you on the streets if something bad happens. Like you need life insurance, you need to insure your house, maybe your car if it's fancy... But not your TV, toaster or phone.

Buy it only if you know you are clumsy and much more likely than the average to break your phone. In general, only insure for things that are very costly to mend if it goes wrong: car, etc

I usually take the insurance for a brief period when it's more likely to break, then cancel it and start paying into my self insurance account. I also self insure for pet medical, travel, etc. remember to pay yourself the premiums!
What don't you just buy the phone outright with your Amex? Purchase protection will cover up to a grand. And it's free.
