Some optometrists allow you to just place an order via them over the phone for a pickup. Some need you to do a vision test(that's covered by your insurance). Also check Eyeconic as others pointed out to see if you can order online.

It takes about $10 in materials, or less, to make the $220 pair of Ray-Ban. I think all the brands you mention are made by a single manufacturer out of Italy. Which also owns half the lens business, too. And half of the chain stores. Also half of internet websites which "compete" with the chain stores.
Yes there is a manufacturer in Italy that produces all brands. Cost is $20 per pair last I heard.
