It is quite uncommon to be rejected. Low levels are essentially never rejected. High levels are given much more scrutiny
Another question, TC is based on interview performance or competition offer? Is it possible that competition offer is too high and they think the interview performance is not good enough to support it?
I’d say about 80% weightage to interview performance and 20% weightage to counter offer

Here's my impression: after HC you go to team matching. Team matching seems like it can take arbitrarily long and I don't think you get "rejected" during this time, but at some point you or your recruiter might give up or move on. Once you have matched with a team and a hiring manager wants you, it does go up the chain for stamping but I think it is uncommon for an SVP to overrule committee and hiring manager. I don't know the exact rate but if I were you I'd not worry about it.
