Str(10) I don't believe that's quite right. For annual adjustment, it's the greater of merit increase due to performance or market adjustment. Unless you're at worst performance rank, that'd put the minimum raise at 2 or 3% IIRC
There are definitely exceptions, though they require high level approvals and really, really good reasons. I've gone through that process to get someone on my team slightly more $$$ than was the max for their band. They were a top performer but not quite ready for a promotion.
